A 25yrs old female came to Dr. Poddar’s Clinic on 27.11.2019 with complaints of patches of psoriasis on elbows, knees with on and off cracks on palms and between toes. She was studying masters in psychology in Gwalior.
She had long history of skin complaints before psoriasis developed. Starting from diaper rash in childhood, being allergic to Johnosns cream with deep cracks in heels at age of 3-4yrs. When she was narrating her complaints, she mentioned of burning in her palms and feet and would wear wet socks during sleep to cool them off.
She was a sensitive person who would write hand written letters on friends birthdays to add personal touch. She was ambitious and would try to achieve what she wants. Though in her training period, she would get referral cases.
Based on the peculiar symptom of her patches and cracks increasing in summer, she was prescribed natrum muriaticum 200. She was observed monthly.
ANALYSIS
Looking for PQRS i.e. peculiar queer rare symptoms is the key. While taking a case pay attention to what patient tells, especially the modalities. In this case when we focussed on summer aggravation, which is not common in psoriasis, the patient showed great relief.
